Third party payment and special funding will be applied whenever possible.

Elder Care Services are initiated by a phone call, fax, or E-Mail to Superior Rehabilitation Services and an appointment is set for the assessment interview. Following the assessment, a conference is held to convey a summary of the client's status, available resources, and options for care. At the client or legal care giver service coordination will be initiated. On going monitoring will be conducted to ensure that the client's needs are met. All requested information is kept in the strictest of confidence.
